The Proba-3 Coronagraph spacecraft experienced an anomaly in February caused by two nearly simultaneous events that locked a software semaphore, preventing reaction wheel desaturation and causing loss of spacecraft orientation. The rare timing condition went undetected during testing and occurred during autonomous operations when no personnel were present at mission control.
