One mistake people make regularly is to change hacked wallets without changing the device.
There are really a few ways to get hacked
Spyware: you click a link, and it installs a spy on your device to see your passwords, etc
Connect to Dapp: A signed transaction may give malicious approval to a contract to withdraw your assets.
Data Leak - Someone saw the seed phrase where it was saved, offline or online.
Example: your data got uploaded to your iCloud or Google Cloud, and your email got hacked, or you saved your seed phrase in your drafts
Apart from a hardware wallet that adds another point of failure, regular wallets are just a pair of two strings: one private, one public, and UI to see the assets and transactions on-chain.
